It looks like a root system issue but not from soil or lack of perlite, more like a genetic issue with that pheno.

Needs repotting now to reduce the leaf yellowing/drop ,it would be rootbound and stressing and nneding to take up more K. Only watering for the first week after re potting. Try canna terra coco and nutes next time and plan to have it in a bigger pot by this stage.

Yes when the tips have rotted due to root rot or high rh, as for the leaf ive seen retarded looking leaves but not missing all together.

Are you sure the ambient temp isnt too cold? when you had success before the nights were probably a lot warmer and clones like warm temps.
