I didn't noticed this when i first looked at the chart. I now see that its just one week 5 plants. Makes a lot more sense now :)

Only other thing i can think of that wasn't said was maybe make each month 2 pages so there is more room for notes and more info.

i would also put month on top right so when you have it in a binder you can see by just flicking corner maybe use left side to list strains and # of plants such #lights etc Just a few more ideas not that you need to use them :P

For PH section i would just leave a blank line to write ph down cause nice to have true ph as 6.2 or 5.8 etc maybe add a little box in corner of each day to mark what day it is. Also maybe section for temp? Looks good though. +rep

How long did you veg for? Roots grow slow when flowing, you want to make sure you have good root mass before you flower. Have you been checking ph and ppm? maybe you have salt build up in your soil? plants look a little stretched, maybe put the light a bit closer.

light doesn't hurt your shrooms or the mycilum. You want to use a non transparent tub to prevent side pinning. A black garbage bag works great as a liner. Side pinning is when the mushroom starts to grow on sides of your substrate and they get all squished up against the side of the tub.
