Can I get some recommendations on a good lp or dispensary to applying?

So canna farms says that the applications forms I was supposed to fill out aren't good enough on top of the doctors fax he sent out whatever that was. Can someone pm me what I'm supposed to send out and recommend a good lp to register for? Im thinking register for canna farms and then register...
