Evaluation Form?

Has anyone created a evaluation form for MJ. Like those sheets for evaluating different wines. Just being lazy. I hate to create one from scratch.
example would be plant info, date started, harvested, cured
taste, buzz- head/body, how long it seemed to increase/ taper off, impressions, etc...

THT, thanks for the suggestion but I wasn't able to locate any electronic canned forms there. If anyone out there has one or knows a thread where someone posted a form example please message me or link in your response.
