You are dealing with multiple deficiencies. I don't usually recommend scraping a plant, because if anything you can use it as a learning experience to identify problems and fix them. In this case I would start all over and brush with sites like Jypsy Dog posted. That plant is beyond screwed.

Unfortunatly it's more a question of what didnt go wrong, that's such a sad plant I'd question everything you've done.

If you list everything then we can help you out.

I'm going to assume bad soil, little or no drainage holes, way over fertilized, and too hot. But those are just guesses without info given.

Def kill it and start over imo
