Long, singular root sticking out of rockwool

I think now is the time to transplant... I have perlite and organic top soil, whats the parts ratio again? And by part, does that mean per gallon? Also, I just plant the rockwool in the soil and thats it right?

I use about 20-25% perlite in my soil, and yes, just make a hole and plant the rockwool into the soil; water thoroughly.

i use 30 t0 35% perlite but i like my soil so the water flows good but will still hold water for a day
