Metal Halide setup question

I just recieved a 400w metal halide bulb and ballas, however, there was no plug coming out from it. on the top there is just a small box with a few leads sticking out of it.
Im not exactly sure what to do with it.

Any help would be appreciated.

you need like a power chord i think, like one that runs into the powersupply/back of a cumputer-unless its exposed wires where then you would have to wire up a chord
