Newgrower Need Help(asap) Please 4weeks Plant Almost Die

can someone tell me what's wrong with my baby??
in her first 2 weeks,she's doing ok but then in 4th weeks it goes suddenly wrong
my baby look like this,it's been a while

Just a guess, but it looks like either over or underwatering. Try backing off on the nutes too.

Just personal experence here but you should try to keep the light closer if it is not to hot. Def on the over watering again from experience i lost 2 plants that way. Are you exhausting that box your using? what type of light also 400w ?
