PATIENT CAREGIVER QUESTION PLEASe HElP

Ok so i have signed up a couple patients before. One had a caregiver and was changing to me so i used the change form. 2nd one he did it as he sent in his paper work for his application, that was easy. Now i have somone that has their card and wants to sign up i have never done this process before. so do i use this form?

only problem is i see is it say required "physician form"
So if its this form then its 100 dollars.

OR is it this form (change form)

Any feed back would be great and i would appreciate it highly.

If you are dealing with an existing patient who is all set and registered and wants to add on a caregiver or replace their current caregiver, you need to use the change form, there's a $10 processing fee I think.

damn you cant be a caregiver with a felony???? thats fucked up...... lol so i guess noone in detroit is is acaregiver.. including the mayor

Most common yes, necessarily going to be a problem? Nah. You can be a sex offender, bank robber, carjacker, or Bernie Madhoff and have the right to grow weed in Michigan. But if you got a felony bust for a gram after getting pulled over in Alabama 30 years ago, you're SOL.

I have a question....I have a friend who sent in his application for a medical card in Michigan and has sponsored me to be his caregiver. I also filled out the paper work and applied as his care giver. He sent it in the mail yesterday, so how long do I have to wait before I can legally start growing for him?
i heard there is a long back log of applications and it may take months or longer before I get a card in the mail.
If it shows that his check for his application was canceled, can I start growing for him at that time, or do I have to wait until I get my card in the mail? That could take years!

Well, they won't process the check until the process the accompanying paperwork. So when the check shws as cleared, they have or are processing. If you start growing upon notification of a cleared check, and are caught, and your application is rejected for any clerical error or other mistake, you are going to be very sorry you didn't wait. Some suggest sending the app certified mail, that way you can show it was received and the check shows it was processed. HOWEVER, that doesn't mean you are approved to grow and supply.
