That map is somewhat misleading.
Ireland has something like one unit of marines that guard the US Consulate here, no bases and no significant permanent troop presence.
It doesn't detract from what you said, but the numbers do seem "sexed up" for a political purpose.

Bias is bias. There are false colonies all over that map.
Ireland isn't unique in that regard.
Our over seas operations aren't very large.
We have a few large naval and air bases.
Outside of the Iraq and Afghanistan infrastructure, of course.
The largest thing we have is the military presence in S Korea and the base in Germany. The German stuff is a fraction of what it was 30 years ago. We're no longer anticipating divisions of Soviet armor rolling in from Eastern Europe.
Outside of those, and seems like one I'm forgetting, we have nothing like an empire.

All things "run better" when the participants individually consent to it. That's not the case in your example.
Without the consent of the individuals involved, your "healthcare" plan must use the same tactic as the military....force.
What you do with the proceeds of the theft, even if it is arguably a "good thing" cannot erase the force used to achieve it.
