A 22-year male presented with recurrent episodes of nausea, refractory vomiting, and colicky epigastric pain for one week. The symptoms were characterized by treatment-resistant nausea in the morning, continuous vomiting, and colicky epigastric abdominal pain. Each episode lasted 2 to 3 h and increased with food intake. He often had two or more episodes a day during the symptomatic period. He had been treated for the severe nausea and vomiting in the emergency room on two occasions in the preceding two months. He also reported having learned to help himself by taking a hot bath each time the symptoms appeared, which dramatically improved his symptoms. This habit had become a compulsion for him for symptom relief with each episode of hyperemesis. On physical examination his mucous membranes were dry, his pulse rate was 102/min and blood pressure was 140/100 with positive orthostasis. The remainder of the physical examination was unremarkable. His complete blood count and comprehensive metabolic panel were unremarkable. In addition, serum amylase and lipase levels were within the normal range. His urine drug screen was positive for tetrahydrocannabinol (THC). Abdominal X-ray series and ultrasonography were within normal limits.

Oesophagogastroduodenoscopy revealed Grade 2 distal oesophagitis and hiatal hernia. On further interviewing, he admitted to consistent marijuana abuse for the past 6 years, often smoking cannabis every hour or two on a daily basis. The patient and his mother did not recall any significant past illnesses or recurrent vomiting when he was a child. He was treated with intravenous fluids with steady improvement in symptoms, and metoclopramide, pantoprazole and morphine for the abdominal pain. It was explained that marijuana was the cause of his symptoms and he was advised not to resume marijuana abuse. On subsequent follow-up, he had abstained from cannabis and remained symptom-free.

Nineteen patients were identified following an original clinical observation by Allen linking chronic cannabis abuse to a cyclical vomiting illness in several cases in South Australia in 2001. Patients were either referred by doctors (12 cases), self referred (two cases), or identified on the ward by the nursing staff (five cases) during acute admission for profuse vomiting. Of these 19 patients, five refused consent and were lost to follow up and 14 fully consented for publication and presentation. Each patient was allotted a letter of the alphabet to preserve anonymity. Patients were followed up with serial urine drug screens and regular clinical consultations to chart their clinical course.

Inpatients were observed with particular reference to autonomic changes in body temperature (measured tympanically), blood pressure, heart rate, fluid intake, skin flushing, and perspiration. However, patient anxiety, compounded by the severity of the hyperemesis, made formal autonomic testing impossible.

All reasonable efforts were made to exclude confounding causes for their cyclical vomiting given the resources at hand. As a result, five patients were excluded from the study for the following reasons:

  • Polydrug use (patients O and C).
  • Porphyria cutanea tarda (patient Z).
  • Acute pancreatitis (patient B).
  • Schizophrenia (patient T).
The remaining nine cases are presented in tables 1▶–4▶. They were compared with a case of psychogenic vomiting described in 1996 by de Moore and colleagues.4

In their article, de Moore and colleagues4 described in detail a man (Mr G), who had smoked marijuana as a teenager, developed a cyclical vomiting syndrome in his twenties, and was noted to have multiple showers on the ward. Marijuana was not proposed as a cause of his illness.

I'm trying to pisstake this but struggling

Australia is generally very anti weed just recently and with thanks to the states

medical weed is coming or there already

and the doctors know their shit

so I give credit to those docs

I fear this could well screw weed in emerging countries,,>

I've had a very mild version where I woke up nauseous and it took awhile for me to feel better. I had a retching impulse once or twice. When a registered nurse friend of mine advised me of the connection, I backed off the constant hitting a pipe or joint whenever wherever and only smoke every few days now on average.

I'm symptom free. Just grumpy cuz I like to smoke lol

Ummmm... Smoking heavily for months or years and then maybe you get a lil sick, and it goes away when you slow down or quit for awhile.

Yeah, I wish alcohol sucked that bad!

yes ... I've been known to exceed my own tolerance

it will explain why I have cranked up the Sauna for the first time in 20 years

the neighbors freaking out as I throw myself in the river at -19c/f

it explains why after eating a large meal I get a kwezzy feeling like hunger in my tum

the odd headache that I put down the CBN/CBH etc

self confessed border line weed addict

I been token daily since 1975, evening is good

so now hospital are gonna want funding for this ...no way

is common sense ?

I think the best move for us old heads is just to slow down on it a bit.

I use it occasionally now and I'm good.

I was suffering all the symptoms until I abstained. But in reality, all medicine has side effects and nausea is probably in the top 3 most common. It should be up to the patient to decide what works for them. JMO.

No. Actually, my wife and I decided that since current circumstances that I have a choice in require that I cessate from alcohol, tobacco and cannabis I was thinking that since cannabis is the easiest to give up I would start there. My wife suggested that I abstain from alcohol first then tobacco. So I quit drinking and am trying to formulate a plan to quit smoking.

I fully intend to continue medicating once everything pans out but hope to never start back with tobacco.

No. Actually, my wife and I decided that since current circumstances that I have a choice in require that I cessate from alcohol, tobacco and cannabis I was thinking that since cannabis is the easiest to give up I would start there. My wife suggested that I abstain from alcohol first then tobacco. So I quit drinking and am trying to formulate a plan to quit smoking.

I fully intend to continue medicating once everything pans out but hope to never start back with tobacco.
I had been a half to a whole pack a day smoker and drank regularly?...few beers after work and get hammered on your off days for the better part of 8yrs. On and off stoner all my life...I picked up a daily cannabis smoking habit about 6 months ago...I don't smoke cigs or drink anymore because I can't stand too. Makes me sick...the more my daily cannabis usage went up the less I desired cigs and booze to the point of nausea
Caffeine too come to think of it...big coka cola fan all my days and all of a sudden can't stand it either...

I drink a bit, maybe one or two daily. I try not to go over three. I smoke when the mood strikes. That used to be constantly, now it's more like once a day or every few days. I've always hated tobacco so I never got hooked on it.

Looks like I'm getting old anyway... Lol

A lot of my friends have struggled with quitting tobacco. All I can suggest is to step down slowly and always resist the urge to step back up again.
My wife knows that I can stop smoking cannabis but has endured my trying to quit tobacco and would rather deal with me sans tobacco but with my medicine than the other way around lol.
