There are several things you might have done wrong:

1) did you keep the soil moist or did you let it dry out? It's best to let it dry out and not keep it really moist because you've already germinated
2) was the pot really cold? i.e. put on a cold floor? It's best to keep the soil in this stage above 70F if possible, but not a complete killer
3) Did this potting soil have lots of nutrients in it burning the root system?
4) did you damage the tap root when placing it down in the soil?
5) did you put it to far down into the soil?
6) people say to put the tap root facing up not down(I've done it both ways and its worked either way), but there may be some validity depending on how deep it is.
7) bad beans?
8) HPS too close and penetrating the soil? You don't need any light until it actually breaks ground, and even then you can keep a 250watt HPS 2' above it seedling for the first week.
9) soil contaminated with something?

There are craptons of things that could have gone wrong ;)

maybe the soil is no good, but never use those jiffy pots again, they are terrible. the sidewalls dry out and kill the roots near them.
use a plastic cup.And it's tip pointed up.
you don't need light on them BTW.

next time plnt the way farmboi says ^

dont grow a very long tap 1/8 to 1/4 then plant like a half inch below soil this way ^

i cover mine wit a baggie and a rubberband or serran wrap keep covered till head showes then i remove cover

an i keep under floursent tubes sum time i switch ti cfls i dont use big lights till transplant @ wk 2 or 3

and i do all on heating pad i normally use pad till transplant to bigger pot

so i use my pad from germ til transplant id say im at about 85 percent

nothing you said you were doing was really bad though so maybe your seeds are duds as mentioned ? My only guess is the soil dried out and killed the root tip once that happens it's game over.

an my way soil wont dry out cause you seal top til it shows its head

when you pot seeds wt soil through the day before dont just put in dry soil an mist top
make sure soil still holdin moisture not soaked

yeah i think the seeds re ok i mean they germed fine but it keeps happening... fairly disheartening tbh. so i should plant them tap root up? :O interesting ill go get new pots and new seeds, im growing in a closet so they are off the floor but i can leave them in a very warm cupboard until they shoot next time easily 70 degrees F, do i need to buy CFL's or can i put them under a 250w from the start but keep the 250 2-3 feet away from them?

i tried the cling film over the top method too under the 250w and those died aswell and i wet the whole cu thouroughly the day before planting also so i really dont get whats happening... mb the soil is bad it does say it has added nutes

cfls are ideal for seedlings but you can use HPS 9 inches is equal to sunlight level, no need to light starve them just because they're babies, in the wild they sprout in full sun.

no you want seed starter soil =nutin in it me ithink its eeasyer wit flour tube den cfl an cfl eayer den hps but all works from easyest to hardest for seedlings

seeddling stage is the hardest stage of a plants life it plant makes past seedling it has like a 95 percent chance to go til harvest

but from germ til first couple of weeks rough time

should be tryin all you can to make this a smooth thing
