the lime I use is powdery instead of chunky so I hate to advise u on an amt to use when I'm not familiar with it. When I use lime I generally just cover the soil evenly in a thin layer and mix. The handful per hole u described sounds about right. Don't include a whole lotta leaf in your mix lots...
