OK but you do realize that those forms are just something some lawyers drafted and have nothing to do with DPH or the MMJ program. Although it certainly cant hurt, It is really a useless form... All that is needed is your Docs letter until May 1st when the rules and procedures are deadlined

i had my doc filled it out sent it to dph cirtified and reg so they would have to sign for it got it back with signature on it a week ago they have my address on it so if im doing something wrong i would think they would of sent the cop by now my cancer doc was all for it why spend 199 for canna to do the same thing i did for nothing those forms are from a mass site with dph address

Must be Cannamed and other procanna docs have special access to forms regular primary care docs can't get. Pretty much all forms are just something written by lawyers. A doctor's rec is a doctor's rec.

See .com at the end there... Not a state site. I understand u can download these forms. I'm saying that they are unnecessary, all that is needed until may is a letter of rec. You hold on to that, and when the state issues its applications through the DPH u can then pay the fee, submit the appropriate application and get your mmj card
