the only thing you need to be a caregiver is for a patient to fill you out as their caregiver on the main form and on a caregiver attestation that you can print from online . There is a few rules like that you must be over 21 and you must not have a criminal record but that is it. You do not need to take a test or see a doctor ever, only a patient needs to see a doctor and get the form signed. I know many people who also went to Dr Graves and they have not had any problems yet from MDCH but at the same time they have not got their cards yet either, they did not receive any denial papers.
Here is how I do my paperwork, I do a lot more then most people some steps I think are unnecessary.
I mail the following with certified mail(so I have proof they received it):
Check for 100$.
ALL MDCH FORMS including caregiver attestation. (with doctors signature and my signature on the attestation, patient needs to fill out his/her portion)
Copy of My photo ID and patients PHOTO ID blown up so it takes up a whole piece of paper (easy to see), Both the back and front of the drivers license.
I keep for myself on file many copies of all this, additionally I have bank statement showing they cashed check and copy of the check and also the certified mail receipt proving they received it.
There is places that offer classes for becoming a caregiver that will help you find a patient but that is totally unnecessary if you find your own patient(s). Likely what happened was some people went to dr graves office and asked to be a caregiver without a patient or something to that effect.
