How do you become a care giver?

I've been on the RI dept of health website. It's pretty confusing because the link to the form that seems to have anything to do with being a care giver, doesn't seem to be right when filling it out. Is the practitioner form the care giver application? Please help?

There are two forms. One is part of the inital patient app, where they can name up to two caregivers at that time. The other form is a 'change' form, allowing the patient to change one or both of their caregivers. As far as I know, there is no form for the caregiver to register themselves...

LOL.... a Caregiver cannot register himself.... Who is he caring for if he does that?? That would be called just a "Marijuana Grower"... If it were that easy, the whole state would be caregivers.... You need to find a LICENSED patient (or soon to be), and have him or her assign you as one of their caregivers.... Thats the only way. Or you can try to go get your patient card, then you can assign yourself as your own caregiver....

I am aware of the patient part. I have a patient with fibromyalgia. She can list me first? I would have thought I would need some sort of license first, get registered, then be listed as a caregiver. When does the $200 fee come into play? Does she list me, then I get contacted by the state? It doesn't really make sense that way, it seems backwards!

If she is a patient already... she needs to get a caregiver application for you to fill out. Then she signs the app and she mails it back to the DOH. You could go get one in person like 2 months ago but now a patient has to ask for one through email i guess... when she mails the application in you include the 200 mail order because its non-refundable. Then you wait 2 - 4 weeks roughly for mail saying denied or accepted an to go in to the DOH for picture id.

The patient must call the Dept of health and request a caregiver add form be sent to them..Then you fill out your info get a BCI and a money order for $200(or $25 if u have medicaid or ssdi) and mail it all in.As long as you dont have a drug felony you ok.

**Drug felony IN RI. You could have moved trailers worth of Heroine in Texas, they don't care,....as long as it wasnt in RI,lol.
