The only real way to protect from it is to refuse all connections
Although, the webserver should be setup to stop high frequency connections from the same endpoint, that should help mitigate the effect.
That's the brute force way of controlling DoS attacks.
There are scalable options as well.

The problem is that most DDoS attacks exploit the underlying TCP handshake protocol so there is no "active connection" to refuse.

Absolutely right. However prior to the point of any active connections being held hostage a well managed proxy on a well balanced cluster can help mitigate those high volume requests. And so in its basic form, you are right, DoS attack connections at some point would begin to be refused. And for god sakes, people need to stop using IIS and/or Apache.
