What system are you using? There's a few different methods for different systems (eg. RDWC , DWC, Bubble pots etc etc)
If it's DWC with a header tank and a fresh water tank, the float in the header keeps the system topped up from your water tank until empty.
RDWC , the same process but you mix your nutes in the header tank and not your Water tank.
Bubble bucket , you'll want to be topping it up yourself wth water to the correct level again. Keep an eye on whether you're consuming more water or food..if they're going down equally you're pretty bang on.
To answer your second question:
Are you talking about in flower? Your nutes shouldn't change, just tail off and then flush.
If you notice an EC decrease, work out how much it's reduced by and take that as a percentage of what to top up on. For example your EC reads 25% less than a week ago, then you add 25% of the dose you administered of each feed type.
This is how I do it.
My nutes are expensive and I never change my system water until time for flower. Sometimes I'll let out half of it and top up with PH,d water and fresh nutes up to the 100% mark again for peace of mind that things are "fresh" but I've never had any problems yet.
