New coronavirus case in California is second of unknown origin

(CNN)An older adult woman from California is the second case of unknown origin of the novel coronavirus, health officials in Santa Clara County said Friday.
The patient didn't have any relevant travel history, exposure to another known patient and wasn't one of the evacuees repatriated from China, said Dr. Sara Cody, director of the county's public health department.
The patient is a resident of the county, officials said.
"This new case indicates that there is evidence of community transmission, but the extent is still not clear," Cody said. "I understand this may be concerning to hear, but this is what we have been preparing for. Now we need to start taking additional actions to slow down the spread of the disease."


The woman had been hospitalized for a respiratory illness and her infectious disease doctor on Wednesday requested a coronavirus test. The case is not related to two others in the county, officials said.

"The virus is here present at some level, but we still don't know to what degree," Cody told reporters.
The CDC this week updated its testing criteria after a Solano County, California, woman wasn't tested for days after being hospitalized. The patient is in serious condition and has been intubated, according to California Rep. John Garamendi.
The Solano County case has puzzled health officials, who say they have not been able to trace where she contracted the virus.
"This does appear to be a person who genuinely did acquire their illness in the community," Solano County Public Health Officer Dr. Bela Matyas said in a news conference Thursday.
Solano County and Santa Clara County are about 90 miles apart. The new patient has not traveled to Solano County, officials said.
The Solano County patient, who is being treated at UC Davis Medical Center, could be the first instance of "community spread" of the virus, the CDC said Wednesday.
The patient was transferred to UC Davis from a Northern California hospital last Wednesday but wasn't tested until Sunday, after doctors at UC Davis persisted in the request. Both hospitals said there was no initial testing administered because the patient didn't fit existing CDC criteria for coronavirus.

That guidance has now changed, CDC Director Dr. Robert Redfield said.
"As soon as that case was recognized, we met and we revised our case definition for persons under investigation," Redfield said. "Today, that has been posted (to the CDC website) along with a new health advisory that the recommendation should be when a clinician or individual suspects coronavirus, then we should be able to get a test for coronavirus."

There are 62 other confirmed cases of coronavirus in the US, according to the US Centers for Disease Control and Prevention.
• 44 are former passengers on the Diamond Princess cruise ship that docked in Japan after an outbreak and quarantine. Two of those cases were newly reported Friday.
• 3 are Americans recently repatriated from Wuhan, China, the center of the outbreak

In the snowy Russian city of Yekaterinburg — thousands of kilometres from the closest international border — Igor Gorbunov says he's doing his part to keep the coronavirus out of the world's largest nation.

"Put this on before you go inside, there might be Chinese in there," he tells a family heading into a grocery store, before handing face masks to a mom and her kids.

Russia has adopted some of the world's most controversial — many would say discriminatory — measures to combat the coronavirus, including policies specifically targeting the Chinese community.

Among them are patrols, such as the one Gorbunov is undertaking, in largely Chinese neighbourhoods.

"Well, we read the press and they write that the virus is most prominent among the Asian population," says Gorbunov's colleague Genady Kovalov. "The danger is real, that's why we started the patrols."

Schools could close and elective surgeries could be cancelled as officials work to stop spread of virus

As global tensions mount over the spread of COVID-19, Canada is crafting its own plan to respond to what could become a pandemic.

The World Health Organization (WHO) has declared COVID-19, a respiratory illness caused by the novel coronavirus, an international health emergency. But so far it has resisted using the label "pandemic" — in part because of the panic that could inspire.

On Friday, the WHO elevated the global risk level to "very high," but Canadian officials maintain the risk here remains low. Fourteen cases have been confirmed to date — seven in British Columbia, seven in Ontario and another unconfirmed case in Quebec.

Officials say preparations are well underway to respond to a global pandemic and a domestic outbreak.

Deputy Prime Minister Chrystia Freeland said that while the government is concerned, Canadians need to take the situation seriously without surrendering to fear.

"I really want people to be confident and reassured that there is a very elaborate, very detailed scenario in place for what we should do," she told host Chris Hall in an interview with CBC Radio One's The House airing Saturday.

Here's what might change if the WHO declares a pandemic, and how Canadians could be affected.

What would authorities do to limit the spread?
Canada's Chief Public Health Officer Dr. Theresa Tam told CBC News Network's Power & Politics that federal, provincial and community health officials have kicked into high gear to prepare for wider transmission of the virus.

Pandemic plans have been developed already and will adapt to any evolving scenario, she said.

"There are going to be some tough decisions that health systems have to make when there are a lot of patients, including prioritizing your beds for those who are most in need. You may need to cancel elective surgeries and that type of thing," she told host Vassy Kapelos.

Infectious diseases specialist Dr. Michael Gardam said a basic premise of any public health pandemic plan is "social distancing," which could include closing schools and universities and cancelling public gatherings.

"They don't stop the virus but they slow it down," he told The House, adding that a delayed outbreak could ease pressure on hospitals already burdened by the busy flu season.

After the 2003 SARS epidemic, which caught many health professionals off-guard, Canada built up public health infrastructure and medical stockpiles to prepare for future emergencies. Gardham said that now makes Canada better prepared than many other countries to respond to a pandemic — with the caveat that most of the country's hospitals are now at 100 per cent capacity.

Could there be community quarantines?
Decisions on how to isolate infected persons will be made by local and provincial health authorities. Their approaches could range from recommended self-isolation, to hospitalization, to quarantine.

Tam said public health systems are adept at assessing and isolating people who are contagious and managing infectious diseases.
