    As close as you can get it without burning the tops of your plant(s). ;) That will allow more light to reach more area, because light loses intensity very quickly the farther away from the source.

    I have seen some auto's only yield about 3-4 grams. They are quite funny to look at, but it depends on the seeds. With those, since they are genetically altered, results can vary widely and you can never really tell what you will get. I wouldn't say it is "normal" per se, but definitely possible...

    I would recommend using the Active Air. http://www.hydrofarm.com/pb_detail.php?itemid=8485 These are really well made if you can get a hold of one at your Hydro Store. I haven't had any issue and these things use LOTS of carbon so you don't have to replace them as often.
