Yes that bump happens when there rooting nothing to worry about and the white fuzzy stuff is the roots they have a smaller fuzzy almost mold like "hairs" coming off them when the bump is large and starts to eat away and go brown its a air embalism

off hand three things come to mind heat, bad bugs in soil, or nutrient lockout. lower the ambient temp or make your heater not blow on the plants, get some predatory nematodes, rinse out your measuring device before adding different nutes. also i have seen heat mats do that to leaves hanging...
