Hello there. Yes, I've had clones die due to being choked from the puck. I keep the clones roughly 1/2 inch into the water in the bubble cloner.. Check this video of an easy cloner construction, this is the method I went with, and it works nicely. http://www.youtube.com/watch?v=1HL-FFH_e78

Yea, totally agree about the node spacing.. I'm currently using cfl lighting, 125w and x6 26w bulbs and I keep them nice and close to the plants and they really love it.
